With health systems striving to meet the triple aim of keeping their populations healthy and satisfied while reducing costs and providing quality care, meaningful patient engagement has become an essential business strategy. At the same time, the explosion of social media and digital tools allowing people to take greater interest in and control of their health – and the mounting consumerist expectation of convenience and service – make the time ripe for providers to rethink and modernize the ways in which they connect with and engage patients. This session will explore the techniques and technologies institutions are using to successfully target populations, tailor messages that resonate, and inspire patients to take charge of their well-being, ideally keeping healthy people healthy and even higher-risk patients with multiple chronic conditions out of the hospital.
